Intro
Whether we are performing diagnostic imaging (ultrasounds, MRIs, or CTs) of your patient or reviewing radiographs, we need you to send us appropriate referral information. Referral information is valuable in helping us get a complete picture of your patient’s current medical condition, to be able to not only contribute to a more thorough and appropriate exam, but also to help make appropriate recommendations to you.
Checklist
• Please send complete referral information for ultrasounds, MRIs, or CTs by fax (802.985.5893) or e-mail.
• Please make sure to send us recent patient history and recent blood work results.
• Recent radiographs that have already been taken are important and helpful. Please send radiographs either digitally (e-mail) or have your client bring them to their appointment.
